The care management and coordination fee supports service planning, coordination, communication, scheduling, monitoring, reviews, and administration of supports for clients whose Support at Home services are managed by I AM PHOENIX.
I AM PHOENIX provides practical in-home and community-based support to help older people remain safe, connected, and independent at home.
Services may include personal care, domestic assistance, laundry support, meal preparation, social support, accompanied activities, direct transport, flexible respite, light gardening, and small home maintenance tasks where appropriate.
Services are delivered by I AM PHOENIX staff or approved support workers. I AM PHOENIX focuses on practical support-worker services and does not publicly advertise clinical nursing, allied health, restorative care management, centre-based respite, group programs, self-managed funding arrangements, or equipment-supplier services through this price list.
I AM PHOENIX accepts Support at Home clients where I AM PHOENIX provides care management and coordination of the client's supports. Service availability, total cost, and service frequency may depend on the client's assessed needs, approved budget, agreed services, travel distance, and service agreement.
